Do you need help to remain independent in your community?
We offer occupational therapy to adults throughout the Yarra Ranges. Our Occupational Therapists (OTs) visit people who are finding it difficult to manage their everyday activities due to age, disability or chronic health conditions, with the aim to maximise their safety and independence.
Our OTs provide advice, education and support in falls prevention, prescribing aids and equipment (such as wheelchairs, bathroom aids, seating, pressure care), home modifications (rails, ramps, bathroom modifications), and training on safe manual handling techniques.
We take a holistic approach to your care. We’ll consider all aspects of what ‘everyday life’ looks like for you and the multitude of things that impact on this. We come to your home so that we can see and fully understand the unique challenges you face. We’ll even help you to connect with other recommended health services as well as navigate and access various funding options for aids that you may need.
Our services include:
Provision of aids and equipment to improve independence
Minor and major home modifications to improve safety and access
Falls education, recommendations regarding the environment to reduce the risks of falls
Assessments and referrals for personal alarms
Assessments and reports to support NDIS eligibility
ADL retraining – teaching people alternative ways of doing everyday tasks to help them to maintain their independence
Information and advice re: general home safety, future planning, external service providers that may assist
