How Do Home Care Packages Work?

Allocation:  If assessed as eligible for a package, you will be assigned a level of care based on your needs. The assigned level determines the amount of funding each individual will receive to access services.

Care Planning: After receiving approval for a Home Care Package, you will work with your chosen Home Care Provider to develop a care plan tailored to your specific requirements. This plan outlines the services and support you will receive to help you remain independent and comfortable at home.

Service Delivery: With the care plan in place, the Home Care Provider arranges and delivers the agreed services. These services may include assistance with personal care, meal preparation, household chores, transportation, nursing care, allied health services, and social support, among others.
